Coverage, nutrition and service quality, measured at the household and the facility

Bharat Survey is a survey agency for public health and healthcare NGOs, CSR health programmes and health departments. We run immunisation and service coverage surveys, child nutrition measurement, facility assessments and community health studies, and we give frontline programme teams an offline Android app for their own monitoring. Consent, privacy and supervised quality checks come before any number reaches a report.

What we run

Health surveys fail quietly when measurement is sloppy: a child weighed on an uncalibrated scale, a vaccine date read from memory instead of the card. The protocol is the product.

Survey agency for public healthcare and vaccination tracking

Household coverage surveys that record vaccination from the card or health record where available, and from caregiver recall only when no document exists, with the source noted for every dose.

Public healthcare and vaccination tracking survey services India

Repeat coverage rounds in programme blocks, reported by age cohort and area so drop-outs between doses are visible, not averaged away.

Epidemiological survey services India

Community prevalence and risk-factor surveys for conditions such as anaemia, hypertension or TB symptoms, designed with your epidemiologist and run under ethics approval where required.

Facility and service quality assessments

Checklists at primary health facilities covering staff presence, drug availability, equipment, cleanliness and patient waiting time, with geo-tagged visit evidence.
Software

For organisations whose staff support community health workers, the app replaces paper checklists and delayed spreadsheets. It supplements government systems; it does not replace them.

ASHA worker survey app for supportive supervision

NGO supervisors use structured checklists during joint home visits with ASHAs: counselling observed, home-based newborn care steps, and referral follow-up. Results show where training is needed, without duplicating official reporting.

Our position on health surveys

Field team training on child height and weight measurement

Measurers practise on children with a trainer, and each measurer's readings are compared before fieldwork begins.

Consent is read in the local language, and caregivers can refuse measurement or any question.

Implausible values trigger same-week re-measurement rather than silent deletion.

Baselines

A health baseline should use indicator definitions that match national survey conventions where possible, so programme results can be read against district and state figures. We agree definitions, age groups and recall periods in the inception note and keep them fixed at endline.

Independent and fieldwork-only roles

Some funders need a third party evaluation agency for public healthcare and vaccination tracking that did not implement the programme; our teams report to the commissioning organisation, not the implementer. Others need a data collection agency for public healthcare and vaccination tracking that runs fieldwork to their protocol and hands over clean data. We state which role we are playing in the contract.

Ethics and privacy

  • Ethics committee review where required
  • Written or recorded consent
  • No names in analysis files
  • Referral protocol for sick children
  • Restricted access to health data
Quality

Measurement

  • Calibrated scales checked daily
  • Standardisation exercise for measurers
  • Implausible value flags
  • Re-measurement of a sample

Interviews

  • Documented source for vaccine doses
  • Back-check visits on a share of households
  • Duration and GPS checks
  • Private setting for sensitive modules

Reporting

  • Denominators always shown
  • Confidence intervals for estimates
  • Limits of recall data stated
  • Indicator definitions annexed

Not the right fit if

  • Clinical diagnosis, treatment or lab testing
  • Replacing government systems such as POSHAN Tracker or HMIS
  • Clinical trials or GCP-regulated studies
  • Publishing identifiable health data

A good fit if

  • Household coverage and nutrition surveys
  • Facility readiness assessments
  • Supportive supervision tools for NGO staff
  • Independent baselines and endlines
FAQ

Straight answers to the questions people ask most about this topic.

Do you measure children's height and weight yourselves?

Yes, with trained measurers, calibrated equipment and a standardisation exercise before fieldwork. Values outside plausible ranges are flagged and re-measured. Children who appear severely unwell are referred following a protocol agreed with the programme before the survey begins.

Does your app replace POSHAN Tracker or government health MIS?

No. Anganwadi workers and ASHAs report through government systems. NGOs use our app for their own programme monitoring, supervision and evaluation, which sits alongside official reporting rather than replacing it.

Do health surveys need ethics committee approval?

Research that collects health information from people often needs ethics review under ICMR's national guidelines, especially with biological measurements or sensitive topics. Routine programme monitoring may not. We flag this during scoping so approvals are in place before fieldwork.

How do you verify vaccination status?

From the immunisation card or health record where the family has one, and from caregiver recall where it does not. Each dose is recorded with its source, and reports show document-based and recall-based coverage separately.
